Patient with strange symptoms

I have a patient who has severe pain in her ear that extends down to the inside of her mouth and causes burning and what seems like nerve pain. She has seen ENT who believes it could be burning mouth syndrome. However, when she travels and is at sea level, her symptoms completely disappear within a couple of hours. Could this be more of a eustachian tube problem? Does burning mouth syndrome often present with ear pain? So far treatment has been gabapentin which is minimally effective so far.

    Up to 25% of atypical or neuropathic pain can be treated with antihistamines, so a trial or two might be useful.
    Etiology: any background issues as allergies, any skin problems, dysautonomia, IBS, joint hypermobility..,?
    Trigger points nearby or occipital?

  2. I'm sorry that your patient is experiencing these symptoms. Unfortunately nothing comes to mind on the side of rare diseases. I hope you are able to find the source and relieve their pain! Thanks for sharing with our community. It must be difficult not having answers.       I've searched our site to see if there is any info from our medical exerts that might assist you, but don't see anything that seems helpful. I hope that perhaps someone on the forum will be able to shed more light on this mystery.
